Friday, March 9, 2007 3:22 PM EST

Campbell County elementary schools will hold kindergarten pre-registration on Thursday, April 19 between the hours of 9 a.m. and 2 p.m. In order to be eligible to attend kindergarten for the 2007-08 session, the following requirements must be met:

l. An official birth certificate must be submitted verifying that the child will be five years of age on or before September 30, 2007.

Parents of children reaching age five after September 30, 2007, and before January 1, 2008, may request that their child be enrolled in kindergarten. The child will be tested to determine readiness for kindergarten.

2. A physical examination report, signed by an authorized medical professional, must be submitted prior to school entry.

3. A certificate, signed by an authorized medical professional, must be submitted verifying adequate immunization as follows: at least three doses of DTP vaccine, three doses of polio vaccine, two doses of measles vaccine, one dose of rubella vaccine, one dose of mumps vaccine, Hepatitis B - a minimum of three doses, one dose of Varicella (chicken pox) vaccine, two - four doses (dependent on age at first dose) of Pneumococcal vaccine, and Haemophilus Influenzae Type b (Hib) vaccine. This vaccine is required ONLY for children up to 60 months of age.

4. The School Entrance Physical Examination and Immunization Certification form (MCH 213E) that is required to be used can be obtained at any Campbell County elementary school.

5. A social security number must be obtained for each child entering school.

We urge that parents with children eligible for kindergarten pre-register their child. Any questions may be directed to the local school principal or to Campbell County Schools.

Pre-kindergarten registration for children four years of age on or before September 30, 2007, will also be held on this date at the following schools: Altavista Elementary School, Brookneal Elementary School, Gladys Elementary School, Leesville Road Elementary School, Rustburg Elementary School, Tomahawk Elementary School and Yellow Branch Elementary School.
